Exit Criteria

Hearts of Gold consumers who meet their goals (as set by the I.D. team), and whose behaviors no longer meet the criteria for our Level-4I home, will be recommended to a less restrictive setting. These successes however, must be maintained for one year.

 

Other reasons to exit our home:

1. The I.D. team may conclude the consumer is not manageable with the staff ratio provided.

2. If the consumer, social worker, family or consumer advocate requests the consumer be moved, Hearts of Gold will comply within 30 days.
